Request an Appointment at Our Family Practice

You can book directly through our practice management software by following the instructions below:

1. Click this link: Request an Appointment
2. Select “Family Medicine” in the Choose Specialty Dropdown
3. Click “Next”
4. Core Medicine of Idaho will appear as your location
5. Click “Search”
6. A screen with our primary care providers and a map will appear
7. Click “Show Available Times” for the provider you choose
8. Indicate whether you have seen this provider at Core Medicine of Idaho before by clicking “Yes” or “No”
9. Click on “Send Request”
10. Enter patient information as requested
    • First Name/Last Name/DOB/Sex/Phone Number/Email
11. Click “Continue”
12. Indicate the reason you are requesting an appointment
    • You can type in your reason for an appointment or choose from the options listed
13. Type any additional information you would like us to know
14. Indicate your preferred appointment information
    • How soon you would like to be seen (ASAP, 1 week, 2 weeks)
    • What day of the week you want an appointment on (Monday - Thursday)
    • What time of day you want an appointment (Anytime, Morning, Afternoon)
15. Click “Continue”
16. Select your payment option
    • Cash
    • Insurance
    • N/A
17. If you would like us to bill your insurance for your appointment, please enter the insurance carrier as well as your subscriber number so we can verify your eligibility
18. Choose your phone number verification method
    • Text or Call
19. Enter the code you received
20. Click “Verify and Book”
21. A confirmation screen will appear saying you successfully requested an appointment
22. Our staff will call you to confirm your appointment and verify your information
23. You will receive a reminder call and text before your appointment

Core Medicine of Idaho requires 24 hours notice for appointment cancellations.

Late cancellations and no shows are subject to a fee.
